How to enter:

1. Download the entry form here and fill in your details.
   
2. Attach a summary of what you are doing during Mushroom Mania ie creative menu, special offers and competitions. Include supporting evidence such as photographs, sample menus or other materials - limit of 4 x A4 pages.

Please note: this competition is not open to the general public.

Judging Criteria

1. Involvement of both front of house and kitchen staff
2. Effort and initiative used to serve more mushroom dishes
3. Promotion of mushroom dishes to your diners.
4. Evidence of extra mujshroom dishes on your menu.
5. Preference for dishes suing more Agaricus mushrooms (white and Swiss Brown buttons, cups and flats).
